[GEMPAK #QHZ-957568]: ncep 4knam grid tables

If you keep a separate dcgrib2 log for the 4km NAM it will report parameters in 
the file which are not decoded because of missing grib tables.  From there we 
can either find existing entries in other tables or create entries for the 
parameter.
