Bjorn, By the way, you may already know this but another "solution" to the Mac case insensitivity is to use a case-sensitive filesystem when you build netCDF on a Mac. Apple has offered the option to make a filesystem case sensitive for several releases of Mac OS X, and many Unix developers who use Macs also use a case sensitive filesystem.
