The information about the #004 GFS for CONDUIT is found here:

To receive the entire 0 hr - 84 hr forecast data set, you would use a request
pattern in your ldmd.conf such as:
request CONDUIT "ST.opnt/MT.gfs" upstream.ldm.hostid
This data is in GRIB2 format.
Other portions of your question regarding general LDM setup and configuration
